The Iberian midwife toad is a robust toad, with wide head and body, prominent eyes with a vertical pupil and short limbs. It presents a white to beige colour, with brown or grey spots, with small red moles on its back. The males carry the eggs on their backs during the mating season, which explains the name of midwife toad. It usually measures less than 4,5 cm in length.
